INSURANCE REIMBURSEMENT
Lice Clinics of America Connecticut is a licensed head lice treatment center. Many insurance companies will reimburse you for some of the cost of your treatment. When you visit, we provide you with the appropriate form including the diagnostic and procedure codes needed to submit a claim for reimbursement,
Your pre-filled Health Insurance Claim Form to file with your insurance provider will be included with your discharge papers. If your claim is denied, you can request a letter of medical necessity from the clinic.
If you need to request a letter of medical necessity, please complete THIS FORM for each individual. We will then submit it on your behalf for creation, and send it to you when complete.
The cost of treatment can often be applied to Flexible Spending Accounts (FSA) and Health Savings Account (HSA) but may not be reimbursable by your insurance provider.
Discounts are available for those patients with Husky Health and Medicaid plans. Please call the clinic for further information.
